A long-term care facility has had several falls during the night shift. The practical nurse is asked to join a quality improvement team. Which contribution is most appropriate?

Coordinated Care on the 2026 PN test plan

Coordinated Care is the largest category on the NCLEX-PN: 18–24% of your scored items. It replaces the RN exam’s Management of Care and tests the practical nurse’s part of running a safe shift: which client to see first, what to assign to assistive personnel, what to report and to whom, when to question an order and how to protect a client’s rights.

What Coordinated Care questions test

  • Assigning tasks to assistive personnel and monitoring the care they give; recognizing which tasks stay with the LPN/VN and which belong to the RN
  • Organizing and prioritizing care for several clients: who is seen first, which finding is reported first
  • Providing and receiving report, and verifying and processing health care provider orders, including questioning an unclear or unsafe order
  • Contributing to the plan of care and collecting client data for the RN and the team
  • Informed consent (the practical nurse’s part), advance directives, confidentiality and privacy
  • Advocacy, client rights, ethics and practicing within the legal scope of the LPN/VN
  • Mandatory reporting of abuse, neglect, gunshot wounds and communicable diseases
  • Discharge, transfer, referral and follow-up; quality improvement, incident data and cost-effective care
  • Recognizing your own limits, responding to unsafe practice by staff and reporting staff conflict

How to study it

  • Learn the three-way split: the RN assesses, plans, teaches first and evaluates; the LPN/VN collects data, gives care, reinforces teaching and reports; assistive personnel do routine tasks for stable clients.
  • For “who first” questions, rank by airway, breathing and circulation, then by what is new or unexpected, then by actual over potential problems.
  • The answer to a scope question is never “do it anyway”: an LPN/VN asked to do an RN task reports to the RN and does the part within scope.
  • Practice the drill until you can say why each wrong option is wrong; Coordinated Care questions are decided by the distractors.

Common questions

What is Coordinated Care on the NCLEX-PN?

The largest client-needs subcategory of the PN test plan, 18–24% of scored items. The LPN/VN “collaborates with health care team members to facilitate effective client care”: assigning tasks to assistive personnel, prioritizing, giving report, processing orders, advocacy, consent, confidentiality, advance directives, legal scope and reporting duties.

What can an LPN/VN assign to assistive personnel?

Routine, standardized tasks for stable clients: vital signs on a stable client, hygiene, feeding a client who swallows safely, ambulation, positioning and measuring intake and output. Data collection that needs judgment, medication administration, reinforcing teaching and any care of an unstable client stay with the licensed nurse.

What is the practical nurse’s role in informed consent?

The health care provider explains the procedure and obtains consent. The nurse may witness the client’s signature, checks that the client is competent and not sedated, and reports to the RN or provider when the client has questions or seems not to understand.
